Checklist for Students Requesting Services

Gather the appropriate documentation.
Provide A or B.

  1. Your most recent IEP, educational and psychological evaluations or 504 plan. It should be no more than five (5) years old. Or
  2. Submit medical documentation from an appropriate health care provider.
    Provide a or b.

    1. Submit the Student Medical Documentation Form

    2. Submit an official letter from a qualified health care provider that treats you for your condition; must be submitted on official letterhead. Student with more than one condition should submit a letter for each specific condition.

      • Every letter should include the following:

        1. Diagnosis
        2. Proof of the Condition – tests that were performed/evaluations; include length of time the student has been in treatment.
        3. Current symptoms and concerns.
        4. Explain how the symptoms related to the student’s medical condition impacts a major life activity (i.e. learning, eating, learning, etc.), in an academic setting.
        5. Recommendation for Accommodation(s).

Student Frequently Asked Questions

Yes. The Office of Accessibility Services is responsible for coordinating accommodations and services for students with disabilities.

No. High schools do not automatically forward IEPs or 504 plans to colleges. The student needs to self-identify and request accommodations, submit documentation and meet with Accessibility Services.

A student needs to self-identify by completing a request form, submitting documentation and completing an initial meeting with Accessibility Services. Accessibility Services will review the information and determine on a case-by-case basis what accommodations and/or services the student is eligible to receive.

You should discuss your current documentation and your needs with Accessibility Services. Usually an IEP and/or a 504 plan alone, does not provide enough information to determine reasonable accommodations.

No, all students enrolled in a class can be eligible, but the student would need to complete the request form, provide documentation and complete the initial meeting with Accessibility Services.

No. Registering with Accessibility Services is voluntary and some students do not request accommodations. Some students do not request accommodations because they either don’t know the services are available or they want to try college without accommodations. Some students may find that they do not need accommodations for all of their classes or every semester. A student can register with Accessibility Services at any time while they are a student at Hudson.

Yes. A student needs to self-identify by completing a request form, submitting documentation and completing an initial meeting with Accessibility Services. Accessibility Services will review the information and determine on a case-by-case basis what accommodations and/or services the student is eligible to receive during the expected duration of the condition/disability.

No. Once you are registered with Accessibility Services you will continue to receive accommodations for the duration of your studies at Hudson. If you need an additional accommodation and your documentation does not support the need, then updated documentation may be requested.

Once Accessibility Services determines your accommodation eligibility, an accommodation letter will be sent to your Hudson email address. The Accommodation Letter will include the accommodations that you need in the classroom that your professor will need to implement in class. You do not need to provide a copy of your documentation to your professor.

Reach out to Accessibility Services to discuss the difficulty you are having with getting appropriate documentation. You may be referred to the Division of Vocation Rehabilitation Services for testing or other local community resources as additional sources for testing.

Accessibility Services (AS) Students Appeal Procedure

The College has implemented this internal appeal procedure for students with disabilities who have followed the proper procedures for requesting accommodations in a timely manner, but believe that they have not been granted appropriate or denied reasonable accommodations, or believe that approved accommodations have not been effectively implemented. The College will attempt, when practical, to first resolve all appeals through its informal process as outlined below. If this does not resolve the issue, the formal process will then be utilized to issue a decision following an investigation. The AS appeal procedure does not supersede or replace other College policies and procedures (grade appeal, etc.). The College prohibits any retaliation against individuals for filing an appeal or grievance.
  1. A student may request that their accommodation request denial or implementation be reviewed and reconsidered by the AS Office.
  2. The appeal should be submitted in writing (see Appeal Form for guidance regarding needed information) to the AS Director, and should be done so as soon as possible after the denial or event serving as the basis for the appeal, in order to ensure a prompt and impartial review into the matter.
  3. The AS Director will review the request and any related documentation, and meet with the student to discuss the appeal.
  4. The AS Director will respond to the student in writing with their decision on the matter within thirty (30) days of the receipt of the request.
  5. If the student is satisfied with the outcome of this process, the appeal will be considered resolved. 
  1. A student who is not satisfied with the outcome of the informal appeal process (above) may file a formal appeal. The appeal must be submitted on the Appeal Form to the Chief Diversity Officer (CDO). The appeal should be filed as soon as possible after the decision/event that serves as the basis for the appeal.
  2. The Chief Diversity Officer will review the appeal, any related documentation, consult with any relevant parties, and meet with the student.
  3. The decision by the CDO will be in writing to the student and delivered within thirty (30) days of the submission of the appeal. The decision of the Chief Diversity Officer is final.
